A US-based medical practice is seeking a Medical Patient Representative to join its patient access team. This is a front-line role that serves as many patients' first point of contact with the practice — by phone, voicemail, and messaging.
Remote Patient Monitoring (RPM) Care Coordinator
Job description
About the Role
A US-based medical practice specializing in sleep medicine is seeking an experienced Remote Patient Monitoring (RPM) Care Coordinator to support patients on CPAP therapy. This role is central to the practice's remote monitoring program: you will review patient data daily, conduct outbound calls to patients who need encouragement or troubleshooting, document interactions accurately for billing compliance, and coordinate with the clinical and DME teams to ensure patients remain on therapy and progressing well.
This is a clinically adjacent role that requires a warm phone presence, strong organizational habits, and comfort working with data in a remote monitoring platform. A clinical background — nursing, medical assisting, respiratory therapy, or similar — is preferred and carries weight for billing purposes as well as patient care quality. Training will be provided on all internal workflows. The practice is looking for a coordinator who can be ready to contribute by November 1, 2026.
Key Responsibilities
Daily Data Review and Patient Flagging
Review patients' CPAP usage data in Sanusom daily, including usage hours, mask leak rates, AHI scores, and missed nights
Identify patients who fall below therapy thresholds or show concerning trends and prioritize them for outreach
Download compliance reports, upload them to the appropriate patient chart in Athena, and assign them to the correct provider for review
Patient Outreach and Coaching
Conduct outbound calls to patients with low usage, elevated mask leaks, or other therapy concerns, using established call scripts and intervention protocols
Troubleshoot common CPAP issues including mask fit problems, pressure discomfort, dry mouth, and air leaks, and guide patients toward solutions
Encourage and motivate patients to remain on therapy in a warm, supportive manner — the goal is to help patients succeed, not to pressure them
Handle a daily outbound call volume of approximately 60 patient calls
Schedule follow-up calls and ensure no patient is lost to follow-up
Documentation and Billing Support
Use Sanusom's call timer to record monitoring time accurately for each patient interaction, in compliance with RPM billing requirements
Document every call in both Sanusom and Athena using standard clinical templates, ensuring records are complete, timely, and accurate
Respond to patient messages via Klara within established response time standards
Care Coordination
Coordinate with the DME and CPAP supply team on mask refits, equipment issues, and supply needs identified during patient calls
Coordinate with scheduling for follow-up provider appointments when clinically indicated
Escalate clinical concerns to the provider promptly and in accordance with established protocols
Systems
The practice uses Sanusom (remote monitoring platform), Athena (EHR), and Klara (patient messaging). Full training on internal workflows will be provided. Prior experience with RPM, RTM, or CPAP compliance platforms is a strong advantage.
Qualifications
Required
Minimum 1 year of experience in remote patient monitoring, chronic care management, or patient outreach — ideally with sleep medicine, CPAP therapy, or DME patients
Solid understanding of CPAP therapy, including common patient challenges such as mask fit issues, pressure intolerance, dry mouth, and air leaks
Warm, patient, and encouraging phone presence — you know how to motivate a patient who is struggling without making them feel pressured or judged
Organized and detail-oriented, with a consistent habit of accurate and timely documentation
Comfortable reviewing data dashboards and identifying patients who need follow-up based on usage trends
Strong understanding of HIPAA and patient confidentiality requirements, particularly in a remote work environment
Reliable high-speed internet connection and a quiet, private workspace suitable for clinical patient calls
Preferred
Clinical background such as a nursing degree (RN or LPN), medical assistant certification, respiratory therapist (RT) credential, or equivalent — this matters for both patient care quality and RPM billing compliance
Experience with Sanusom, Athena, or Klara
Prior experience in a sleep medicine practice, DME company, or CPAP compliance program
Familiarity with RPM and RTM billing codes and documentation requirements
What We Are Looking For
CPAP therapy is only effective when patients actually use it — and many patients struggle in silence without the right support. The RPM Care Coordinator is the bridge between the data and the patient: you see that someone has not been using their equipment, and you pick up the phone. The quality of that call matters. Patients need to feel heard, not lectured. They need practical help, not a checklist. And they need to know that someone at their practice actually noticed.
The right candidate for this role combines clinical awareness with genuine interpersonal warmth. You are organized enough to manage a high daily call volume without losing track of anyone, disciplined enough to document every interaction correctly for billing, and motivated by the knowledge that your work directly improves patient health outcomes.
